All orders are due the first Sunday of the month. Cards can be picked up the third Sunday of the month, from 9am – 12pm, or call me to make other arrangements.
Instructions for filling out order form
- Choose the items and quantities you want to purchase. Multiply the quantities by the dollar value of the card you want. This is the amount you owe for that item. For example: You want 2 – $25.00 Dominick’s cards. Your total owed would be $50.00. The discount does not apply to you. It is the amount Manna will give Trinity on your order. Do this for all of the items you want to order.
- Total all items per column and enter in the subtotal and total boxes on the bottom of the form.
- Write a check for the total amount payable to: Trinity United Methodist Church
- Attach the check to the order form.
- Make sure you fill out your name, phone number and email address on the top of the form.
- Due date is the first Sunday of the month. Either:
- Bring your order and check to church. I’ll be there between 9:00am and 12:00pm to collect them. If you don’t see me there, you can drop them into our new secure Manna Box in the Narthex.
- Mail it to me at the address on the order form. Allow that it has to reach me by the first Sunday of the month.
- Drop it off at my house.
- Pick up your order at church on the third Sunday of the month from 9am – 12pm, or call me for other arrangements.
